A Case of Endogenous Klebsiella Endophthalmitis Associated with Acute Urinary Tract Infection, Cellulitis in a Type 2 Diabetic Patient

Abstract
Endogenous endophthalmitis is a rare intraocular inflammation, caused by the migration of the pathogen from a distant primary site of infection to the eye where it crosses the blood-ocular barrier. Endogenous endophthalmitis caused by Klebsiella pneumoniae was considered a particularly devasting disease, characterized by preexisting illness and poor visual prognosis. We report here a case of endogenous endophthalmitis due to K. pneumoniae associated with acute urinary tract infection and cellulitis in a patient with type 2 diabetes. A 61-year-old man visited local hospital for evaluation of headache and blurred vision. Four days later, he was referred to our hospital and admitted. K. pneumoniae were isolated and identified from from the culture of blood, urine and vitreous fluid. The patient was started on combination antibiotics by both intravenous and intravitreal route. The sign of endophthalmitis got progressively worse despite treatment and enucleation was performed 8 days after admission.
